2. Course Name (Ritual, Relief, Glow - Facial Lift or Skin Detox)
3. Duration (50, 80, or 110min)
4. 3 preferred Time-Slots (1. MM/DD, 2. MM/DD, 3. MM/DD)
5.  Please let us know if have/are
       a. Pregnant (Yes or No)
       b. Skin trouble (Yes or No)
       c. Anything you would like us for to know in advance
